Program Coordinator - 160007QZ Job Program Services Coordination/Case Management Primary Location United States-NJ-NJHackettstown Organization Hastings Description New Jersey MENTOR is seeking a Program Coordinator to provide assistance to individuals with disabilities in a community-based setting. Program Coordinators facilitate the implementation of Individual Service Plans (ISP) and participate in a range of administrative duties including staff hiring, training, scheduling and oversight. In addition, Program Coordinators may provide support to individuals receiving services, including: assistance with daily activities such as meal preparation and planning; medication administration; and personal care. Program Coordinators may also facilitate community involvement by accompanying individuals on outings in the community or providing transportation to work or other activities. Qualifications Bachelors degree in social/human services related discipline with one year relevant experience. Must be 18 years of age Ability to apply common sense understanding to carry out instructions furnished in written or oral form. Ability to deal with routine problems involving several concrete variables in standardized situations. Must possess current driver’s license in good standing and may require registration and insurance on personally owned vehicles used to transport.

Successful clearance of two (2) reference checks, local criminal background checks and successful fingerprint clearance and any further checks required by state regulations.
