We are seeking knowledgeable, dedicated and compassionate RNs to provide direct care to hospice patients in their homes, assisted living and / or nursing facilities. As a Hospice RN, you will work with other healthcare professionals to determine the best overall care for your terminal patients (prognosis of 6 months or less). Additionally, you would provide emotional support to the patients' families. This position offers potential for growth, a friendly staff, and an opportunity to interact with patients and their families on a more personal level.
 
Please note: This is a full time, field based position that requires travel throughout a portion of our service area which may include. This position requires the RN Case Manager to work 1 day per week 10 am - 7 pm. This position also requires 1 weekend day per month (10 am - 7 pm).  Participation in a rotating on call schedule is also required - which will include weekends and holidays.
 
Primary Responsibilities:


Conducting regular visits at home or nursing facilities for a caseload of hospice patients
Coordinating, assessing, and implementing patients' care
Providing accurate and timely documentation of patient services to reflect plans of care
Overseeing and participating in day to day clinical care of terminal patients and their families
Attending weekly interdisciplinary meetings
Interacting with other group members such as CNA's, LPN's, social workers and dieticians
Performing physical activities that require considerable use of your arms and legs and moving your whole body, such as climbing, lifting, balancing, walking, stooping, and handling of biohazard materials
Participating in a required on call schedule that rotates throughout the team which includes weekends and holidays

Requirements

Required Qualifications: 



RN licensure (current, unrestricted) in the state of Virginia

2 years of clinical experience

Access to reliable transportation that will enable you to travel to client and / or patient sites within a designated area

Ability to perform physical activities that require considerable use of your arms and legs and moving your whole body, such as climbing, lifting, balancing, walking, stooping, and handling of biohazard materials.
Preferred Qualifications:

Computer proficiency with MS Office products including Word and Outlook
Acute care experience
Hospice experience
Palliative care experience
Case management experience
Home care experience
Long term care experience
Proficiency with electronic medical records (EMR) software
