Hospice Aide responsibilities and tasks are according to the plan of care are as such, but not limited to:Providing care consistent with written instructionsAssisting with personal hygieneAssisting with ambulation and exerciseReporting changes in the patient’s condition and needs to the RN Case ManagerProviding nutritional support by assisting in oral feedingPerforming other related supportive tasks as specified by organizational policyCommunicates with the Hospice RN Case
Manager regarding services provided and significant findings regarding
the patient’s functional status and change in care needsDocuments the patient’s response to care in compliance to the Evercare Hospice and PalliativeCare Policy and Procedures at the time services are providedSubmits documented visit records and
completes all job duties in a manner that meets the accepted standards
of practice and the Optum Palliative and Hospice Care Policies and
ProceduresParticipates with the on-call rotations as assignedAttends and participates regularly in Hospice Staff meetingsContributes to achieving the company’s missionWorks collaboratively with others to achieve goals

Requirements


Required Qualifications: High School Diploma or GED 1+ years of CNA experience CNA in good standing in CA CPR certification or the ability to obtain certification within 30 days if hiredAccess to reliable transportation that will enable you to travel to client and/or patient sites within a designated areaComfortable with 100% local travel to homes and facilitiesPreferred Qualifications: Hospice Care experiencePhysical Requirements and Work Environment:Must be able to lift, turn, bend, and stoopCareers with Optum.
