*** Monday through Friday work schedule; 8am to 4:30pm  (Hours to include 2 days late days per month.  10:30am to 7pm in your time zone) *** 
 
As a Maternity Case Manager, you will review patient status over the phone while ensuring they are receiving the best care. In this role you would be working with members in various stages pregnancy.  
 
Primary Responsibilities:  


Making outbound calls to assess members' current health status
Identifying gaps or barriers in treatment plans
Providing patient education
Interacting with Medical Directors on challenging cases
Coordinating care for members
Making referrals to outside sources
Coordinating services as needed (home health, DME, etc.) 
Encouraging members to make healthy lifestyle changes
Documenting and tracking findings
Making post - discharge calls to ensure that discharged member receive the necessary services and resources
   

Requirements



Required Qualifications: 

Current, unrestricted RN license
3 + years experience working with maternity / OB population
Computer proficiency, to include strong data entry skills and the ability to navigate a Windows environment (Microsoft Word, Outlook, and Internet)
Preferred Qualifications: 

Nurses with a compact state license
BSN
A background in managed care
Case or disease management experience
Experience/exposure with discharge planning
Certified Case Manager (CCM)
