As an Integrated Health Service Care Manager, you will be responsible for planning, implementing, and evaluating integrated medical, behavioral, dental and pharmacy programs for adults, children, families, and seniors in the Arizona Medicaid program.
 
The Integrated Health Service Care Manager will coordinate with the Business Partners who perform clinical functions to meet regulatory requirements. Includes: preparing activity reports to inform management of the status, services, quality initiatives, and outcomes.
 
This is a office position at 1 East Washington Street, Phoenix, AZ
Primary Responsibilities:

Carries out responsibilities in accordance with the organization's policies and regulatory requirements
Monitor, evaluate and communicate process improvements
Serve as a liaison and provides support to Business Partners and internal Health Service team
Assess barriers to care and assist members to address concerns
Provide support to clinical teams and collaborate with business partners, health plans, hospitals, vendors, providers, family, support network
Develop workflow activities & activities for designated programs
Provides education and training for business partners and team members
Represents the department in assigned Plan committees/meetings
Supports excellent employee performance and participates in staff development
Provide support to the Health Services Director
Assist in program development

Requirements

Required Qualifications: 

Current, unrestricted RN license in the State of Arizona
5+ years clinical, behavioral or case management experience
Case Management experience
Ability to prepare responses for regulatory submissions
Intermediate level of experience with Microsoft Word and Excel, with the ability to navigate a Windows environment
Self-directed in work case load
Preferred Qualifications:

Bachelor's /Master’s Degree
A background in managed care
Experience/exposure with discharge planning
Medicare / Medicaid experience
A background working with the AZ Regional Behavioral Health Authorities
Strong organizational skills and multitasking abilities will be keys to success
