Services are provided with the recognized standards of practice, South Dakota Nurse Act, and the patient’s Plan of Care that encompasses physical, educational,  ain/symptom management needs of patient/family/caregiver. The Plan of Care is based on the initial and comprehensive assessments of patient/caregiver. On-Call rotation is Required.

  • Responsible for all nursing interventions with emphasis on teaching and relief of pain/symptoms.
  • Completes and documents accurate and complete comprehensive assessment/OASIS/, admissions, transfers, discharges and Plans of Care (POC) given to designated person within 2 working days. Daily visit documentation is completed at Point of Care, paper-clipped together with DAR (Daily Activity Report) and placed in the designated receptacle or faxed by M the next working day.
  • Updates the Plan of Care (POC) to include disease- specific signs and symptom interventions with measurable goals that reflect the changing needs of the patient that is consistent with their current needs. The POC is reflected in the visit notes. Comes prepared for efficient, concise POC discussion and scheduling at Interdisciplinary  eeting and encourages team ideas for POC accomplishment.
  • Demonstrates the ability to appropriately assess patients, apply clinical interventions, evaluate outcomes, and implement change to the plan of care.
  • Handles unanticipated events and crisis situations in a professional manner. Is alert to potential problems and notifies appropriate individual as necessary. Maintains competency of the “on call” role and responsibility by consistently working required call schedules.
  • Facilitate efficient, appropriate, coordinated care to ensure positive patient outcomes.
  • Works cooperatively with all members of the healthcare team by expressing a customer service approach for both external and internal customers.

Required Licensure - Certification - Education - Experience

  • Graduate of an approved school of nursing.  Associates Nursing Required; Bachelors Nursing Preferred
  • 1 year in Hospice or Home Health preferred
  • Current SD Registered Nurse License
  • American Heart Association Certified
  • Current Driver’s License
  • Auto Insurance to meet minimum state requirements
  • Personal vehicle
