The RN Case Manager will join the family of dedicated nurses and other health care professionals living out a mission to heal people in body, mind and spirit. In collaboration with the Director of Case Management and Manager of Care Coordination, the Case Manager will be responsible for collaborating with healthcare providers and physicians for problem solving, prioritizing and facilitating patient care objectives, the coordination and development of staff education, and discharge patient education and follow-up for the area assigned.

The Utilization Review Nurse will be responsible and accountable for unit systems functions, staff performance, and patient care outcomes. The Discharge Planner / Case Manager will assist Director and Manager of Case Management in accomplishing unit and organizational goals.

The RN Case Manager will coordinate and manage the care of patients to facilitate the achievement of quality and cost effective outcomes. The Case Manager will work in collaboration with both internal and external interdisciplinary staff and participate in quality improvement activities as they relate to the management of patient care.

The ideal candidate for RN Case Manager should have two years recent acute clinical experience with discharge planning, utilization management and critical pathways preferred. Effective written, verbal and time management skills. Ability to work independently as well as establish relationships and function within a team. Knowledge of medical information systems. Excellent customer service skills required.
