What You Will Do:

As a Care Transitions Coordinator you will serve as a clinical liaison between health care providers to ensure continuity of care for patients transitioning from a facility to a home care environment. You will build and maintain professional relationships with key referral sources in the medical community.

* Visit the patient onsite to review and assess the patient's clinical needs and gather clinical information from the patient and the physician. The Care Transitions Coordinator also facilitates patient involvement in his or her own care by providing education and obtaining the necessary information required for successful transition to home.

* Develop the referral relationships of the care center within the community by forecasting needs, setting priorities and executing strategy while supporting the philosophy, objectives and goals of the care center.

* Collaborate with the patient care team to provide assistance in obtaining information from referral sources and handling stressful situations and deadlines by effectively communicate with all members of the healthcare delivery team.

* Participates when appropriate in patient care team conferences at the agency to ensure patient/client satisfaction and continuity of services delivered.

* Participate in community activities, network groups and related home health professional organizations as the Agency representative; serves on committees as requested/required.

* Plan and execute special events which serve to advance staff, patient and community relations including home care trade shows, health fairs, etc while using organizational skills in a fast-paced environment.

Required:

Must meet one of the following qualifications:

* Bachelor's degree in business, management, nursing, or healthcare with one (1 ) year experience in sales. (OR)

* Three (3 ) years' experience in healthcare, sales or related industry (OR)

* One (1 ) year experience within the last year selling to referral sources or building relationships within the healthcare industry.

AND

* RN, LPN, PT, OT, SLP and PA are considered in some cases (with a current, active, unencumbered license in the state of service)

* Proven track record of developing and maintaining relationships within a competitive marketplace

* Must have reliable transportation, current driver's license, and required liability insurance.

Preferred:

* Registered Nurse

* Experience in home health business development

* Bachelor's degree in business, management, nursing, or healthcare
