Client Care Manager – Part Time - RN required

We produce great results because of great people.  We are looking for a passionate, energetic candidate with a love for working with seniors, and assisting them with healthy and happy living in the comfort of their own home for our Client Care Manager position. We are currently seeking a passionate individual to work 24 hours/week who can lead a very interactive environment through constant change and growth. This individual will specialize in putting the CARE into our CAREGivers through sharing knowledge and expertise in patient-centered care.  They will manage all hospital and skilled nursing facility discharges to our agency to ensure superior transitional care back into their home.  They will also build skills, increase loyalty and strengthen our customer service techniques to provide the highest quality service to our clients. 
 
Responsibilities:
  • Manage, coordinate and track all quality assurance visits by the QA team
  • Develop and educate CAREGiver training for red flags based on conditions
  • Oversee all personal care skills performed by the franchise office’s CAREGivers.
  • Identify opportunities for healthcare training modules
  • Implement Hospital to Home tool kit
  • Coach and mentor other staff trainers for growth and development
  • Participate in CAREGiver performance reviews and identify opportunities to grow talent; Promote development of CAREGiver skills both personally and professionally
  • Manage CAREGiver relationships to foster retention and loyalty
  • Track all required hospital and Skilled Nursing Facility discharge and readmission data
 
 
 
Qualifications:
  • Registered Nurse in State of Michigan Required
  • Prior management and leadership experience preferred
  • Demonstrates high initiative, problem solving and multi-tasking/organizational skills
  • Must demonstrate excellent oral and written communication skills and the ability to listen effectively
  • Must have the ability to work independently, maintain confidentiality of information and meet deadlines
  • Must demonstrate effective interpersonal skills as well as sound judgment and good decision-making skills
  • Must demonstrate discretion, integrity and fair-mindedness consistent with office standards, practices, policies and procedures
  • Must demonstrate knowledge of the senior care industry
