Primary Responsibilities:

Engage
members either face to face and over the phone to educate about their
healthReview
available member service records and relevant documentation (e.g.,
utilization history, functional level, stratification information, current
plan of care)Conduct
comprehensive member assessment that includes bio-psychosocial,
functional, and behavioral health needsIdentify
when supplemental assessments are needed and conduct supplemental
assessments Identify
member service needs related to health concernsIdentify
urgent member situations and escalate to next level when necessaryEngage
member to participate in the assessment process and collaboratively
develop plan of care based on their individual needs, preferences, and
objectives with nursing oversightWork
with members to develop healthcare goals and identify potential barriers
to achieving healthcare goals

Requirements

Required Qualifications:

High
School Diploma/GEDComfortable
working Monday-Friday 8 hour shift between hours of 7am - 7pm, overtime
possible depending on business needsCPR
Certification or the ability to obtain within 30 days of hireAccess
to reliable transportation that will enable you to travel to client and/or
patient sites within a designated areaHigh
Risk Maternity and/or High Risk OB experience 1+
years of telephonic customer service experienceMust
have resided within the community for 1+ yearsAbility
to create, copy, edit, send and save utilizing Microsoft Word, Excel, and
OutlookAbility
to have a home office

Preferred Qualifications:

Bachelor's
Degree (or higher) in Social Work and/or Health Care AdministrationExperience
working in the Behavioral field with Substance Abuse and/or Mental HealthExperience
working in managed careKnowledge
of Nevada Medicaid & Medicare populationWorking
experience in a medical office or other provider environmentMedical
terminology knowledgeKnowledge
of Medicaid health care and insurance industryExperience
working within the community health setting in a healthcare roleField
based experienceFamiliarity
with the resources available in the communityBilingual
with English and Spanish
