Job Summary:
Reviews physician referral and patient's condition and medical records to determine treatment plan, plans and prepares written treatment program based on evaluation of patient data, evaluates effects of treatment at various stages and adjusts treatments to achieve maximum benefit, records treatment, response, and progress in patient's chart or enters information into the computer, instructs patient, family, and staff in treatment procedures to be continued at home or by restorative therapy, demonstrate competence in managing Medicare A, RUG category resident's care, and other duties as assigned.
Essential Job Functions/Responsibilities
* Provides physical therapy services to patients according to a written physician's plan of care.
* Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.
* Prepares and submits clinical and progress summaries based on the attainment of goals.

Education
Possesses a degree from a baccalaureate, Masters or Doctorate program in physical therapy approved by an accredited organization.
Experience
Two years of appropriate experience as a physical therapist. Community/home health experience is preferred. Demonstrates good verbal and written communication and organization skills.
License/Certification
Licensed to practice as physical therapist within the State of service (MN, ND, &/or SD).
Possesses and maintains current CPR certification. Current Driver's License required.
