Position Summary

Provides direct patient care and documentation of that care as outlined in the policy and procedure guidelines. Evaluates needs of patient/families, plans treatment programs, and implements appropriate physical therapy modalities within the multidisciplinary team approach.

Essential Job Functions

  • Completes initial evaluation, progress notes and discharge summary per department time frames.
  • Coordinates the plan of care in therapy cases with ancillary staff and community resources.
  • Coordinates care plan schedule and visits needed with supervision and other physical therapy staff, especially during times of high census. Supplemental staff may be used when agency staff’s schedules are full.
  • Discharge planning is begun upon admission. Discharge plans show coordination with physician, patient/family, and community referral resources, as evidenced by chart review.
  • Participates in ongoing multidisciplinary team to identify problems, establish overall treatment objectives, develop care plans, direct and provide patient care, and perform discharge planning for Home Health clients.
  • Provides appropriate treatment and evaluates progress of Home Health/Hospice patients and in-home outpatients on an intermittent basis, as indicated on the care plan and progress note, completed after each visit, and submitted within one working day, as observed by chart review.
  • Submits all reports and documents necessary for timely billing: admission data within one working day, travel and activity report within one working day, and updated recertification forms within ten working days before the end of certification period.
  • Understands internal and external customer satisfaction principles. Promotes programs and services such as “On Your Feet” and back protection with community, staff, and clients.

Education/Experience/Licensure/Certification

Bachelors degree in Physical Therapy Required

Current South Dakota license as a Physical Therapy Required

2 years experience in healthcare/medical field as a Physical Therapy preferred

Lymphedema certification and Kinesiotaping training optional
