Items needed for Delivery Driver approval
- Current Motor Vehicle Record (DMV report) - 6months - 1yr intervals
- Proof of Insurance (front page of policy) - when vehicle or policy changes or expires
- Vehicle Inspection Form - as needed when changing or adding vehicles
- Signed Delivery Driver Agreement
- Smith System Defensive Driving Instruction Course must be completed
- Pizza Delivery Exclusion Acknowledgement (If driver has "Mercury Insurance")
- Fax all info to Lorraine Knox at HRH/Summit prior to approving driver to deliver orders for Round Table Pizza
- Driver cannot drive until approved by HRH/Summit
- Driver must notify management immediately if they are involved in an accident; no matter who is at fault
Insurance Requirements
- (CA State Law) California requires that drivers and vehicle owners carry the following minimum monetary limits:
- $15,000 for injury or death of 1 person per accident
- $30,000 for injury or death of 2 or more persons per accident
- $5,000 for property damage per accident
- Evidence of financial responsibility must be carried at all times in the vehicle
- Round Table Pizza is not responsible for accidents; you need to report this to your insurance company
- State of California’s minimum
- limits are 15/30 and this is all we can require
- Proof of insurance and drivers license will be validated every 6 months
- A new vehicle inspection is require if drivers change or add vehicles
- Check with your car insurance company to verify that they cover your vehicle while delivering orders for Round Table Pizza
Safety/Accident reports
- Delivery Driver Safety/Observation Report - to be used if managers note unsafe driving (for personnel file)
- Delivery Driver Accident Report - to be used to report accidents. Needs to be completed in full and faxed to Naida Abel at HRH Summit and Maryann Graf at corporate if driver is involved in an accident
- Round Table Pizza is not responsible for any damages to your car while “on” or “off” the clock; any damages to your car must be reported to your insurance company
- Never allow anyone else to accompany you in your car while delivering orders (unless for training purposes approved by your manager)
- It is important to state that drivers must report accidents to their own insurance company first (in addition to reporting to HRH/Summit and Maryann Graf in corporate)
- Keep your doors locked at all times and do not leave your car “running” while making deliveries to the customers door
- Wear seatbelts at all times
- Obey all traffic signals
- Slow down when approaching schools, parks and residential areas for small children
Workers Compensation
- You will be covered under our workers compensation insurance if you are injured on the job while driving on the clock
- This does not cover your vehicle or medical treatment for the individuals that were involved in the accident
Signage
- Your manager will discuss with you the requirements by Round Table Pizza for requiring signage on your vehicle while delivering orders
Tip Reporting
- It is the employees’ responsibility to report their tips daily when “cashing out” their orders
- Keep a daily log of the tips you make for reporting purposes
- Failing to report your tips will subject you to IRS audit rulings
- You will be “taxed” on your tips through your paycheck
