The Director of Clinical Pharmacy reporting to the VP/Medical Director will be responsible for the implementation of specialized AppleCare Medical Group pharmacy programs related to pharmacoeconomics, utilization management, quality metrics and anticoagulation.  Specific programs include Chemotherapy and Injectable review, Polypharmacy, Medication Reconciliation, Medication Adherence and Anti-Coagulation Clinic. They will work collaboratively with the AppleCare Medical Management Chief Medical Officer, Medical Director, Care Management Team, Quality Team, Health Plans, Information System staff and physician providers.
 
Primary Responsibilities:

Leadership role in the planning, development and implementation of pharmacy programs that positively impact clinical, quality and utilization metrics
Review and determine authorization for all injectable requests including but not limited to chemotherapy and biologics
Provides pharmaceutical information to AppleCare physicians and specialist providers, such as generic or other alternatives
Handle drug utilization review calls from physicians
Participate in interdisciplinary teams
Research medical literature and/or clinical guidelines
Develop and implement projects in conjunction with Quality Improvement, Care Management or other teams
Work collaboratively with Care Management to intervene on high risk members or high cost pharmacy utilizing members
Perform post-discharge medication reconciliation
Polypharmacy management
Participate in Joint Operating and Medical Management meetings
Work collaboratively with Health Plans on Pharmacy related projects
Review pharmacy contracts and assist with negotiations
Prepare and analyze pharmacy utilization reports
Review and intervene on pharmacy related claims issues
Management of members’ anti-coagulation therapy

Requirements

Required Qualifications:

BS Pharmacy or Pharm D
Active, unrestricted pharmacy license in CA
Strong working knowledge or experience with managed care or health plans
3+ years of experience as a working Pharmacist
Knowledge of Commercial, Medicare and Medi-Cal benefits structures
Proficient in Microsoft Word, Excel, Outlook, PowerPoint
Experience producing meaningful analytical management reports
Strong communication and project management skills
Ability to effectively conduct meetings and present information
Ability to perform complex problem solving
Strong interpersonal skills and ability to work effectively with physicians
Ability to lead people and manage multiple priorities
