Our Primary Care Center is a leading source of pediatric primary care in Connecticut, and home to the largest pediatric primary care practice between New York and Boston. More than 13,000 children from Hartford and surrounding neighborhoods receive their primary care in Connecticut Children's Primary Care Center. Clinical services of the Primary Care Center include health supervision, behavioral health care, chronic disease management, adolescent medicine and urgent care using an NCQA Level 3 recognized patient-centered medical home model. We serve as the referral site of choice in the region for children with complex medical and behavioral health conditions, and serve predominantly children from low income families and disadvantaged backgrounds. The Primary Care APRN or PA delivers comprehensive quality patient care, including diagnostic and therapeutic interventions according to standards of practice to children from birth through adolescence. Responsibilities include: * Manage a panel of patients and promote preventive health care through health supervision visits, patient and family education and child health advocacy. * Treat and evaluate patients with acute and chronic childhood illnesses and promote the health of the child and family. * Coordinate patient care with team members, other departments and community agencies and providers as necessary. * Provide inpatient care for newborns in the neighboring Hartford Hospital newborn nursery. * Participate in departmental quality improvement and peer review activities. We are seeking a pediatric APRN or PA, with post-graduate clinical experience, preferably in a primary care setting. Newborn nursery experience is also highly preferred. Some weekend nursery coverage required. Our Primary Care Center is located in a beautiful, new facility on New Britain Avenue in Hartford. Registered and Certified Physician Assistant or Pediatric Nurse Practitioner with a minimum of two years' experience in pediatrics required. Registered and Certified Physician Assistant -or- Current State of Connecticut Physician Assistant licensure -or- (Pediatric) Advanced Practice Registered Nurse (APRN) licensure required at time of hire and maintenance at time of appointment or privilege approval (allow one-year grace period for renewal if gap.) based on CT Children's bylaws. For PAs hires, requires to obtain and maintain NCCPA certification at time of appointment or privilege approval; allow one-year grace period for renewal if gap. Current Basic Life Support (BLS) American Heart Association accredited is required upon hire. Specialty licensure/certification may be required, as specified by department.
