LOOKING TO HIRE IMMEDIATELY
Reports To: Office Manager
Purpose: A patient care coordinator facilitates the delivery of care by the Provider within our Electronic Health Records (EHR), CureMD. Patient Care Coordinators (PCC's) are liaisons to the patient/family/caregiver, works with the Providers direction and orders to provide quality healthcare for the home bound or limited patient.
Primary Duties/Responsibilities:
o Answer phone calls/messages from or pertaining to Grace At Home patients.
o Complete daily follow-up sheets including ordering of labs & tests, referrals, ordering of durable medical equipment (DME) & phoning in prescription refills
o Scheduling & routing of existing/new patients for designated provider (daily, weekly, monthly)
o Provide support to designated provider in all patient care areas
o Management of required signature documents
o Processing of all completed documents (including outgoing faxes)
o Dropbox/VPN Provider "out" box management (pertaining to signed documents)
o Track outgoing referrals to home health agencies.
Other Duties:
o Communicating & collaborating with other PCC's to assure all work completed
o Updating and maintaining good working relationships with Home Health agency staff
o Updating resource information
o Maintaining good working relationships with Facility staff
o Keep work space within HIPPA compliance
o Communicate daily with Provider to encourage delivery of comprehensive care plan to each patient within the team and expedite work flow to facilitate Providers ability to see patients
Tasks
o Check EHR email (approximately every 2 hours). Follow up on requests timely.
o Answer all incoming phone calls if possible. Check voicemail (VM) every hour and return phone calls within 2 hours. VM should be cleared out up to 4pm phone calls daily. (Only VM received after 4pm should be left to the following business day).
o Reconcile completed signature paperwork in providers "out" DropBox/VPN daily.
o Return all signed home health documentation within 1 business day of completion by provider.

Must be a multi-tasker, customer service oriented, organized, prioritize, must be able to handle fast paced environment, team player, exhibit empathy and be willing to go that extra mile to help our patients. We work with the geriatric population so patience is a must.
