Under general supervision of the Director of Clinical Pharmacy, the Clinical Pharmacist is responsible for performing comprehensive and targeted Medication Therapy Management (MTM) in high risk patients to identify, resolve, and prevent medication-related problems. The Clinical Pharmacist documents the care delivered and communicates essential information to the patient’s primary care providers and the health care team. The Clinical Pharmacist is also responsible to follow processes to improve patient care, outcomes, and quality improvement metrics related to CMS HEDIS/Star ratings and assist in the coordination of the Medical Assistance Program (MAP) for WellMed patients.

Please note: This is a full time, office based position located at our WellMed office on NW Parkway in San Antonio.  1 – 4 hour Saturday shift is required every 4 weeks.

Primary Responsibilities:
Utilize prescription and medical claim data to generate clinical recommendations according to Medication Therapy Management and Retrospective Drug Utilization Review program protocols
Provide clinical recommendations pertaining to, but not limited to, gaps in care, high risk medications, compliance and adherence, drug interactions, therapeutic substitution, and generic substitution
Utilize health plan’s formulary information to guide appropriate medication recommendations
Knowledgeable of current and emerging clinical trends
Active participation in various clinical departmental meetings and forums to improve clinical programs, enhance processes, and share clinical information
Continuously look for ways to streamline and improve workflow
Assist with user acceptance testing for new clinical programs and system enhancements
Serves as a clinical resource and consultant in all areas of pharmacy and medication management
Assist in mentoring Pharm.D pharmacy students
Acts as a consultant to WellMed Anticoagulation Clinic
Supports medical management and pharmacy quality initiatives

Requirements
Required Qualifications:
Doctor of Pharmacy degree
Current Pharmacist license to practice in the applicable state or the ability to provide proof of licensure within 90 days from date of employment
2 or more years’ experience as a clinical pharmacist
Excellent written and verbal communication skills
Excellent relationship building skills
Planning and organizational skills to demonstrate leadership and initiative
Strong interpersonal skills. Ability to work independently, as well as a member of a team
Strong analytical and PC operation skills
Experience working with provider relations, working within various payor environments, and risk management
Ability to travel as necessary
Preferred Qualifications:
Fluent in Spanish Language a plus
Board certification by the Board of Pharmacy Specialties (BPS)
Certified Geriatric Pharmacist
Certification in Medication Therapy Management (MTM)
 Experience with Medicare guidelines
