RN Case Manager - Ft Myers, FL - Full-time / Part-time

Please note: This position will require onsite visits at an assigned hospital(s) in Ft Myers, FL. The RN Case Manager will work from home when not attending onsite visits at the hospital. Primary Responsibilities:


Making outbound calls to assess members' current health status
Visiting members in the hospital to prepare for transition home
Identifying gaps or barriers in treatment plans
Providing patient education to assist with self - management
Interacting with Medical Directors on challenging cases
Coordinating care for members
Making referrals to outside sources
Coordinating services as needed (home health, DME, etc.)
Educating members on disease processes
Encouraging members to make healthy lifestyle changes
Documenting and tracking findings
Making "welcome home" calls to ensure that discharged member receive the necessary services and resources

Required Qualifications:

Current, unrestricted RN license in the State of residency
5+ years’ experience as an RN
Experience working with clients in the community setting
Demonstrate a proficiency in computer skills including Microsoft Word, with the ability to navigate a Windows environment
Must have a designated work space and access to install secure high speed Internet via cable / DSL in home
Preferred Qualifications:

Bachelor's Degree
A background in managed care
Case Management experience
Certified Case Manager (CCM)
Experience / exposure with discharge planning
Strong organizational skills and multitasking abilities will be keys to success
