Responsible for providing routine and complex wound care to various types of patients seen by this service in accordance with established Clinical Practice Guidelines. Responsible for total patient care, including venipunctures, ear irrigation, invasive and non-invasive monitoring, quality control of equipment, wound care, accurate and timely record keeping. Manages patients' plan of care collaborating with the staff and other disciplines within the unit (i.e., completes/Coordinates patient assessment, Develops/monitors outcomes based on plan of care, Coordinates and Implements plan of care, evaluates outcomes and monitors variances).

Provides hands-on expert care to patients and Accountable for managing patient care in order to meet optimal outcomes. Responsible for cross training to provide delivery of acute and chronic wound care, and admission assessment in accordance with Clinical Practice Guidelines.

Minimum JobQualifications

MinimumEducation: Associate's Degree - Nursing degree from an accredited nursing school; Registered Nurse/RN; Must complete Basic wound care course within six months of employment

Minimum years in fieldprevious to employment: None required

* Able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit as described in unit-based competencies, policies and procedures

* Demonstrated critical thinking and independent decision-making

* Good communication skills with the adult and older adult population

* Familiarity with PC's and computerized nursing systems and tools, or demonstrated ability to learn

* Strong interpersonal skills to interact positively and effectively with patients, visitors, staff, and physicians

* Good organization, delegation and communication skills to facilitate efficient patient care

* Strong analytical decision making and critical thinking skills

* Good leadership ability and coordination skills

* Completes Basic wound care course within six months of employment

* Familiarity with a variety of patient care equipment; 2. Must have manual dexterity and ability to perform difficult fine hand manipulation skills such as IV insertion

PositionResponsibilities

1. The RN delegates and collaborates with other staff members

2. Collaborates and communicates with physicians to ensure quality care

3. Develops, Utilizes, and models effective time management skills

4. Plans for appropriate patient populations, and Manages plan of care with interdisciplinary team through ongoing collaboration and use of care plans

5. Participates in development of collaborative care

6.Coordinates discharge and outpatient plan of care with patient's family

7.Collects data to utilize and analyze outcome-based care management variance reports to evaluate patient progress

8.Communicates with all team members regarding therapeutic interventions including patient/family/significant other

9.Sets patient care priorities

10.Responsible for total patient care, including venipunctures, ear irrigation, invasive and non-invasive monitoring

11. Delegates appropriate tasks to licensed staff, monitoring and maintaining accountability for the quality of care

12. Utilizes interdependent team decisions making to ensure quality of care

13. Develops and Coordinates the education plan appropriate to the patient/significant other family learning style. Communicates plan to patient/significant other/family and or interdisciplinary care team members

14. Manages resources, which promote cost effectiveness, efficient operation of unit, and positive outcomes

15. Performs comprehensive patient assessments, anticipating potential care problems and prioritizing care needs in order to effectively provide optimum care

16.In collaboration with staff members, Provides bedside patient care, implementing the interventions identified in the plan of care

17.Document patient's plan of care and interventions, according to policy

18.Initiates and communicates change in patient care delivery based on continual evaluation of progress according to outcome-based care plan

19.Provides input into employee feedback process for the other members of the staff including the Medical Director

20. Initiates collaboration with physician and interdisciplinary team members by effective communication of the pertinent patient information to insure continuity of care

21. Participates in education and training for the staff; coaches and Provides feedback on an ongoing basis to all team members; Serves as a mentor for the staff

22. Make recommendations to improve patient care delivery process

23. Acts as a clinical resource to manage difficult patient/family situations

24. Maintains and demonstrates competency in BLS (Basic Life Support), infection control, safety and all unit-required skill review

25. Utilizes interventions substantiated by research as appropriate to the practice environment
