Overview

The registered nurse provides nursing care in accordance with the orders of the patient physician and the policies and procedures of the VNA Division and accepted standards of practice. Performs related duties as assigned.

Responsibilities

Deals with all individuals with a spirit of friendliness and undivided attention. Demonstrates compassion for the sick, poor and uneducated. Implements and honors patient rights and responsibilities. Exhibits behavior that is indicative of an understanding of Hospice philosophy and mission and nurtures that behavior in Hospice clinical staff.Demonstrates knowledge of and ensures compliance with federal and state regulations, appropriate accreditation standards, St. John's policy and procedure, Home Care policy and procedure, St. John's HospiceCare policy and procedure. Ensures the safe delivery of HospiceCare services. Maintains an appropriate level of confidentiality toward position. Demonstrates effective verbal and non-verbal communication.Admission and on-going paperwork is completed correctly within appropriate time frames, with at least 90% compliance as per chart audits. IDG plan of care content and measurable goals are appropriate on-going. IDG plan of care is updated as needed and at least every two weeks.Ensures coordination with Referral Intake staff regarding evaluation of patients for admission including physician orders, six-month terminal diagnosis, and financial reimbursement. Coordinates the admission process for hospice patients in a timely manner. Completes pre-admission evaluation of patients for acceptance according to St. John's HospiceCare policy and procedure. Coordinates the admission process within time frame established per national standards and St. John's HospiceCare protocol. Assigns patient case managers and interdisciplinary team upon admission according to availability and/or geographic area. Coordinates and initiates physician orders for admission to St. John's HospiceCare. Provides on-going communication with the Hospice Clinical Manager related to program operations and the admission process.Responsible for completing or correcting the initial plan of care for St. John's HospiceCare patients according to St. John's HospiceCare policy and procedure. Provides effective communication of patient goals, interdepartmental needs, customer needs, and other aspects of the admission process. Uses the computer as appropriate and transfers information on a daily basis, ensuring that all assignments, orders, and careplans/guidelines are complete. Appropriate patients-related communication is found on all patient charts (per audit).Attends/reviews appropriate meeting minutes for IDT and staff meetings, with at least 90% attendance/review of staff and nurses meetings and the Hospice Happenings. Participates in required in-services and competencies related to professional practice (per competency plan).Daily itineraries are completed correctly and in a timely manner. Personnel file documentation is current and timely.Coordinates referrals to interdepartmental services according to the initial plan of care both verbally and in writing. Communicates effectively with patients/families regarding requests for follow-up with interdisciplinary team members.Obtains approval for admissions form the Director of HospiceCare regarding private insurance admission, non-reimbursed services, pediatric admissions and other special populations, and special needs of patients meeting the criteria for acceptance. Coordinates and communicates with Hospice Case Manager any requested insurance case management updates. Demonstrates a working knowledge base regarding guidelines for reimbursement for Medicare, Medicaid, and private insurance.Assesses caregivers on effective coping and makes referrals as needed. Routinely assesses the medication refill needs of patients and arranges for delivery as needed. Coordinates medical equipment services as indicated on the patient's plan of care. Identifies appropriate safety issues per safety assessment and provides appropriate related intervention.Makes appropriate nursing judgments - demonstrates appropriate nursing skills (supervisory visit/incident reports). Practices nursing in a manner that at all times protects the safety of patients and their families (supervisory visit/incidents). Patients and their families verbalized satisfaction with their nursing care (supervisory visit). Maintains established productivity (weekly report/itineraries) per program standards. Demonstrates effective verbal and non-verbal communication.Participates in on-call schedule as required (call schedule). Makes after hours/weekend visits as appropriate (on-call records). Accepts extra call/visits to meet agency needs. Willingly accepts and effectively performs additional duties as assigned.Participates in quality improvement processes, committees as assigned, and community activities as appropriate and/or assigned. Participates in required in-services and annual competencies related to professional practice. Assists with the implementation of customer service initiatives. Meets established personal and professional goals.Meets Health System attendance standards. Wears proper personal protective equipment when appropriate. Observes standard precautions consistently. Wears clothing and Health System badge consistent with dress code, and attends to personal hygiene to maintain a clean, well-groomed appearance.

Position Qualifications:

* We require a nursing degree from an accredited school.

* You'll also need a current RN license in our state.

* Basic life support training should be completed in 90 days with advanced training required at certain intervals.

* Preferred Experience: Home health or Hospice experience preferred.
