POSITION SUMMARY
Please note that this Registered Nurse (RN) - Field Case Manager position requires travel within a ~50 mile radius of current residence. Looking for candidates in multiple geographies. In order to be considered for this position, you must hold an active Registered Nurse (RN) license. Full Time work at home yet with daily travel out in the field up to 75%.
Fundamental Components of the Field Case Manager role include, but are not limited to:
The Registered Nurse (RN) Field Case Manager utilizes a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individuals benefit plan and/or health needs through communication and available resources to promote optimal, cost-effective outcomes. This will be accomplished through telephonic outreach along with face to face visits if the member meets criteria and lives within a 50 mile radius from the case managers home/office.
Additional responsibilities include:
* Utilize clinical skills in a collaborative process to assess, plan, implement, coordinate, monitor and evaluate options and services to facilitate appropriate healthcare outcomes for members.
* Collaborate to assess, plan, implement, coordinate, monitor and evaluate options and services to meet an individuals health needs through communication and available resources to promote quality, cost-effective outcomes.
BACKGROUND/EXPERIENCE desired:
* Minimum of three years case management experience.
* 5 years clinical practice experience.
* Ability to travel within a designated geographic area for in-person case management activities
* Knowledge of laws and regulations governing delivery of rehabilitation services preferred
EDUCATION
The minimum level of education desired for candidates in this position is a Associate's degree or equivalent experience.
LICENSES AND CERTIFICATIONS
Nursing/Certified Case Manager (CCM) is desired
Nursing/Registered Nurse is desired
