JOB SUMMARY

The Sepsis Program Coordinator is a health care professional who works in collaboration with the medical and hospital staff to provide multidisciplinary care to the septic patient. The Sepsis Improvement Coordinator provides clinical, professional expertise for patients, families and staff members. The Sepsis Improvement Coordinator facilitates the bedside clinical care of sepsis to assure that all patients receive comprehensive and timely treatment interventions that are consistent with national best practice recommendations through program development and establishment of evidence based processes. This position collaborates with all members of the healthcare team through consistent daily clinical patient rounds to assist in the planning, implementing, monitoring and evaluation of care delivery for sepsis patients.

QUALIFICATIONS

EDUCATION

* Bachelors in Nursing required.

* Masters in Nursing preferred.

EXPERIENCE

Required:

* Experience in acute care setting with direct care of critically ill patients in either ED or ICU.

Preferred:

* Experience in quality improvement.

LICENSURE AND CERTIFICATIONS

Required:

* Registered Nurse - State Licensure -- RN

Preferred:

* Certified Emergency Room RN - CEN

* Certified Critical Care RN - CCCRN

SKILLS AND KNOWLEDGE

* Lean (preferred),

* PI strategies and familiarity with SSC sepsis guidelines (preferred),

* Experience in project management (preferred)

* Familiarity with A3s,, value stream mapping (preferred);

* Education materials development

* Proficiency in spreadsheets, email, navigating health system style programs,

* Familiarity with EPIC is preferred
