Client Care Coordinator Job Description
- Must have a Registered Nurse license issued from the State of NJ
- Responsibilities:
- Perform Nursing assessment of each client and develop a individualized care plan
- Perform a 60 day monitoring visit in home to ensure quality of care
- Meet with potential clients and family members to discuss their needs
- Perform an in-home re-assessment at 6 months to re-evaluate the client needs
- Supervise Certified Home Health Aides in the field
- Demonstrate open and effective communication with clients, family members, the franchise owner and colleagues
- Work with other team members to coordinate various aspects of a client's care
- Oversee discharge of clients
- Be available by phone when on call to mange CHHA problems with clients
