Responsibilities and Qualifications

Assistance is provided by using the appropriate policy, protocols, standards of practice within the PCA scope of practice. Recognizes and reports abnormalities in patient condition or monitored data to theappropriate staff. Provides patient monitoring according to policy and protocol, identifies behavioral health issues and responds in therapeutic manner. Recognizes limitations, and seeking additional assistance if necessary.The Plan of Care is supported by admitting patients and completing the appropriate forms. Reports pertinent information to staff and collaboration with the healthcare team to plan/provide patient care as evidenced by documentation.Care is provided by assisting patients with activities of daily living in a timely and caring manner. Documents the patient's response to procedures, therapies, medications and interventions. Insures patient safety through monitoring of patients and environment. Demonstrates or developing proficiency in performing and/or assisting in procedures and therapies according to policy and procedures.Documentation is provided by ensuring that the patient record reflects the plan of care, patient care delivered and the status during his or her care and discharge. Ensures that the information is legible, concise, accurate, objective and timely, while meeting hospital standards.Leadership is provided by directing and coordinating assignments and care given by team members based on staff competencies, experience and scope of responsibility. Participates in the PI process and identifying opportunities to improve patient care delivery, including collecting data, monitoring and implementing changes.Enhances the learning opportunities to other staff, and by effectively contributing to the orientation of team members and students. Exhibits dependability by scheduling and working shifts according to applicable policy. Exhibits dependability by limiting absences and tardiness in accordance to HR policy.
