About this job

Currently seeking a RN Hospital Liaison. The RN Hospital Liaison performs initial assessment visit in the in-patient facility and develop plan of care in collaboration with physician and patient. Coordinates initial delivery of medications, services and supplies. Works in collaboration with account executive & sales/marketing team to educate potential referral sources and develop new business.

Schedule:

Monday - Friday 8:30a-5:00p
Over time, occasional weekend or holiday may be required.

Main Responsibilities (including, not limited to)

  • Collaborates with doctors, patient and caregivers to develop plan of care
  • Coordinates start of service with appropriate intake and clinical staff at Home Infusion Pharmacy and other external agencies to facilitate the process.
  • Meets with patient and family to educate about home infusion services and performs initial assessment of home care needs, including medications, skilled services, and supplies.
  • Completes necessary admission documentation
  • Discusses funding sources with patient/family as appropriate.
  • Serves as Home Health Service (HHS) representative in communicating information related to HHS services, policies, billing, etc., to all necessary parties
  • Maintains patient load and establishes new business by developing and maintaining professional relationships with physicians, nursing agencies, hospital staff and other referral sources and educating referral sources. (20-25 patient load)
  • Participates in the development of new programs and services and educates referral sources regarding those services.
  • Maintains records of referral information as requested.

Qualifications

  • Active RN license
  • A minimum of 1 year experience in IV therapy or Home Infusion Services
  • Excellent organization and communication skills
  • Discharge planning experience preferred
  • Ability to use (or learn) a tablet and cell phone
