Position Purpose:

Sanctuary at Saint Mary's is currently recruiting for full time 3rd shift Certified Nurse Aides. We offer full benefits, tuition reimbursement, uniform voucher and much more!

Under the direction of the Registered or Licensed Nurse Manager/Designee, the Certified Nursing Assistant/Geriatric Nursing Assistant will provide assigned residents with routine daily nursing care and services in accordance with the resident's assessment and care plan.

Job Description Details:

1. Know, understand, incorporate, and demonstrate the Mission, Vision, Values and Guiding Behaviors of Trinity Health and Trinity Senior Living Communities in behaviors, practices, and decisions.

2. Consistently demonstrate (leads by example) Sanctuary Values to all internal and external customers (residents, visitors, volunteers, and associates.) Create and maintain an atmosphere of warmth, propagating a calm environment throughout the community.

3. Execute assignments in a culture that is shared and collaborative across all divisions within TSLC.

4. Perform all assigned tasks in accordance with our established policies and procedures, and as instructed by your supervisors. Follow work assignments, and/or work schedules in completing and performing your assigned tasks.

5. Supports the admission, transfer, and discharge process for the Resident.

6. Assist Elders with daily living functions such as dental care, feeding, bathing, dressing/undressing, nail care, hair care, and shaving according to expressed preference.

7. Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.

8. Perform restorative and rehabilitative procedures as instructed. Provide daily Range of Motion Exercises. Record data as instructed. Answer Resident calls promptly.

9. Follow established safety precautions in the performance of all duties (i.e., exposure to body fluids, lifting, transferring, use and disposal of personal protective equipment).

10. Wear and/or use safety equipment and supplies (e.g., gait belt, mechanical lifts, etc.) when lifting or moving residents in accordance with plan of care.

11. Report all changes in the Resident's condition to your supervisor/designee as soon as practical.

12. Maintain the confidentiality of all resident care information. Ensure that all nursing care is provided in privacy.

13. Enters information and documentation accurately in Care Tracker in accordance with company policy, practice and standards.

14. Follow established End of Life Procedure.

15. Attend and participate in scheduled training, in-service training, mandatory annual in-service training and educational classes as required/needed.

16. Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the community policies and procedures.

MINIMUM QUALIFACTIONS

1. High School Diploma or equivalent.

2. Must possess a current, unencumbered, active certification to practice as a Nursing Assistant in this state or certificate of completion of state approved nurse aide training program.

3. Minimum of one-year experience within a healthcare setting preferred.

4. Previous experience working in long-term care or with the geriatric population preferred.

5. Must possess superior customer service skills and professionalism.

6. Must possess outstanding verbal and written communication skills.

7. Ability to adapt to ongoing change and work in a fast-paced, customer-driven environment

8. Possesses interpersonal and relationship building skills to work in a cross-functional team as well as the ability to

work with residents based on their cognitive ability.

9. Self-starter, highly motivated with a high productivity level.

10. Possesses a high degree of personal accountability, responsibility and strong decision making abilities.

11. Must possess the ability to plan, organize, develop, implement and interpret programs, goals, objectives, policies

and procedures of the organization.

PHYSICAL AND MENTAL REQUIRMENTS AND WORKING CONDITIONS

1. Must be able to adapt to frequently changing work parameters.

2. Must be able to see, hear and smell or use prosthetics that will enable these senses to function adequately so that

the requirements of this position can be fully met.

3. Primarily works inside with frequent kneeling, crouching, reaching, lifting, sitting, walking, bending and lifting.

Must be able to push, pull, move and or lift a minimum of 25 pounds to a minimum height of 3 feet and be

able to push, pull, move and or carry such weight a minimum distance of 10 feet. May be necessary to assist in the

evacuation of residents in emergency situations.

Physical mobility that includes movement from place to place.

Physical agility, that includes ability to maneuver body while in place.

Dexterity of hands and fingers.

Coordination, including eye-hand, hand-foot.

Must possess the above ability with or without the use of prosthetics that will enable adequate functionality so that the requirements of this position can be fully met.

4. Subject to exposure to noise, infectious waste, diseases, conditions, etc. including TB, HIV, HEP B viruses. May

be subject to the handling of and exposure to hazardous chemicals.

5. Must be able to speak, read and write the English language in an understandable manner.

6. Must be able to relate to and work with ill, disabled, elderly, emotionally upset and at times hostile individuals within the community. Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.

7. Must not pose a direct threat to the health or safety of other individuals in the work place.

8. Must be able to cope with the mental and emotional stress of the position.

9. Must meet the general health requirements set forth by the policies of this community, which include a medical and physical examination.

10. May be required to work beyond normal working hours, on weekends and other positions temporarily when necessary and may be subject to call back during emergency conditions. May be required to work on shifts other then those which originally hired.
