Responsibilities: The Midwest Admission and Transfer Center (MATC) nurses must have a high acuity background with excellent triage and assessment skills. The MATC nurse is at the center of the patient transfer requests for Mayo Clinic Hospital- Rochester and multiple Mayo Clinic Health System hospitals, and patient safety must be the focus of all interactions. They will assess and triage patient clinical information as shared by the referring provider and utilize process flows, emergent and/or specialty algorithms and admission guidelines to assign patient at the right facility, at the right level of care with the right admitting service. The MATC facilitates conference calls between the referring provider and accepting physician. If patient concerns are identified, they communicate these concerns to the multidisciplinary team, including the referring provider and the receiving nursing unit for patient safety. The goals for the Midwest Admission and Transfer Center are: identification of the right patient; obtaining the right clinical information; determine the right level of care; the right accepting Mayo Clinic hospital and right accepting physician service. Determining the right level of care may necessitate the involvement of specific emergency telemedicine services. The MATC activates and supports emergency telemedicine services for specific patient populations. The MATC nurse maintains a comprehensive view of the patient admission and transfer process including helping to identify emergency transport needs and has a close partnership with the Emergency Communications Center (ECC) to expedite helicopter, fixed wing and critical care transport needs and requests. Qualifications: Student or Graduate of an accredited baccalaureate, associate, or diploma nursing program, as recognized by the Accreditation Commission for Education in Nursing (ACEN- formerly NLN) or Commission on Collegiate Nursing Education (CCNE). If graduated from a nursing program that was not accredited by ACEN or CCNE, at least one year of RN experience (within the last 5 years) in an applicable care setting is required. If graduation did not occur within the last two years, one year of RN experience in an applicable care setting is required or, effective October 1st, 2017 one year of current LPN experience at Mayo Clinic is required.

Rochester campus: All entry-level associate degree registered nurses with a RN start date after September 1, 2010, must provide documented evidence of program completion of their baccalaureate degree in nursing from an accredited nursing program within 6 years from the last day of the month of their RN start date. All entry-level associate degree registered nurses with a RN start date of September 1, 2014 and after must provide documented evidence of program completion of the baccalaureate degree in nursing from an accredited nursing program within 5 years from the last day of the month of the RN start date.

Additional Qualifications: The Midwest Admissions and Transfer Center RN will provide symptom-based triage and assessments over the phone. This is a non-direct patient care role. RN's hired into this role must have extensive experience in a high acuity setting with strong clinical judgement, critical thinking skills, attention to detail, and strong communication skills. Applicants with experience in phone triage, ED, ICU, telemetry, or high acuity procedural areas will be strongly preferred.

License or Certification: Must obtain prior to start date:

Current RN license by applicable state requirements.

Midwest: Basic Cardiac Life Support for healthcare providers- BCLS -- certificate of course completion required and must not expire within 3 months after start date. A scanned copy of the certificate of completion or transcripts, including your full name, must be attached to your profile to be considered.

* American Heart Association (Title: BLS for Healthcare Providers or BLS Provider)

* Red Cross (Title: CPR/AED for Professional Rescuers and Health Care Providers or BLS for Healthcare Providers)

* Military Training Network

Additional specialty certification/training as required by the work area.

Compensation Detail: Education, experience and tenure may be considered along with internal equity when job offers are extended. The minimum pay rate is $32.72 per hour.

Hours/Pay Period: 48-72 hours per 2 week pay period

Schedule Details: Sunday -- Saturday; Nights, 12-hour shifts -- NOT NEGOTIABLE

Weekend Schedule: Every third weekend -- NOT NEGOTIABLE
