Job Summary
The Public Health Nurse (PHN) is responsible for the delivery of patient care and grant programming within the scope of professional nursing standard. The PHN uses the nursing process (assessment, planning, intervention, and evaluation as a systemic approach to the direct and indirect care to clients and families.
Job Responsibilities
* Deliver public health and case management services to clients using initiative, judgment, and independent decision making within the scope of professional nursing standards.
* Complete grant workplan activities based on the grant requirements
* Utilize policies and procedures when performing assigned activities
* Chart according to grant and community health guidelines
* Direct and guide the activities of other health personnel
* Attend required trainings/meetings
* Link the client with appropriate providers and resources throughout the continuum of health and human services and care settings.
* Provide culturally sensitive, evidence-based health education to clients and families
* Bachelor's Degree in Nursing required.
* Current MN RN License required.
* PHN certification required.
* Adult health experience required.
* Pediatric health experience preferred.
* Public health experience preferred.
* Must be self-directed and able to function independently.
* Must have current MN driver's license.
* Excellent communication and customer service skills.
* Basic computer skills.
* Able to meet ADA requirements.
