This position is at Northeast High School Clinic within the School-Based Health Clinics, the mission of the School Based Health Services Program is to assess, protect and promote the health of children and adolescents, helping to assure that they are healthy in the classroom and ready to learn. Staff deliver effective, efficient, and quality healthcare on site and provide the infrastructure necessary for treating acute and chronic diseases; provide care for acute illnesses; work towards alleviating co-morbidities among at-risk high school students; deliver culturally competent primary preventive, and emergency health care; and improves the overall health status in a substantially uninsured and medically underserved population. Services are provided by a multi-disciplinary team of health and mental health professionals and paraprofessionals which include a pediatrician, advanced registered nurse practitioners, health support technicians, registered nurse, a licensed practical nurse, licensed mental health therapists and clerical support.

The Advanced Registered Nurse Practitioner is responsible for providing comprehensive health care to clients, including diagnosing and treating acute, episodic, and chronic illness and helping to prevent diseases and injuries as part of a medical home team. Monitoring and oversight includes an established quality improvement process with ongoing chart review, including peer review.

Essential Functions:

Ensure the provision of care to patients with medical problems. This includes the recognition of acute symptoms, management of emergencies, discrimination between normal and abnormal findings on physical assessments, reporting of findings, and appropriate decision making. Provide routine treatment in collaboration with physician(s) based on policies, protocols, and practice agreements.

Ensure the development and implementation of a plan of care for each patient including: additional diagnostic studies, if indicated; treatments, including prescribing medications within protocols; referral to other health care providers and for social services, as appropriate; teaching, counseling, assisting patients and other significant persons to assume responsibility for self-care, prevention of illness, promotion/maintenance/restoration of health; follow-up to assure continuity of care.

May be assigned to other primary care clinic areas, as necessary, and when the SBHC is closed.

Remain up to date on issues pertaining to school-based health center services.

Practice accepted infection control measures in accordance with the OSHA blood borne pathogens standards, and Center policies and procedures.

Attend all meetings and/or training sessions as required.

Perform other duties and projects as assigned.

Analyze and interpret patient histories, symptoms, physical findings, or diagnostic information to develop appropriate diagnoses.

Develop treatment plans based on scientific rationale, standards of care, and professional practice guidelines.

Recommend interventions to modify behavior associated with health risks.

Educate patients about self-management of acute or chronic illnesses, tailoring instructions to patients' individual circumstances.

Instruct on topics such as health education and disease prevention.

Participate in quality assurance activities including meetings and peer review.

Comply with HEDIS measure, Meaningful Use standards, productivity standards, and selected goals for quality of care.

Performs other related duties as assigned by supervisor.

MINIMUM QUALIFICATIONS:

2 - 3 years of ARNP experience

1 year of electronic health record experience

Clear and active license in the state of Florida to practice as an ARNP

Current CPR Certifications
