POSITION SUMMARY
This position is for an experienced clinical nurse who works within the Community Disease Management Ministry providing community members with access to health and wellness services to promote community disease management. This includes but is not limited to health screenings, education, and self-management programs. This role will work alongside the Community Outreach Manager and Coordinator in developing, implementing, and conducting the outreach plan for the overall market including but not limited to the expansion of a Congregational Health Program and Mobile Health Unit. Additional responsibilities include maintaining relationships and collaborative initiatives with internal and external stakeholders with the goals of increasing utilization of primary care, reducing risk factors for chronic disease and health disparities, and empowering individuals to make healthy lifestyle choices. This position requires competence in management, technical and clinical skills, as well as cognitive knowledge.
JOB REQUIREMENTS
1. Graduate of an Accredited School of Nursing/Current licensure in the State of Maryland
2. Bachelor's Degree preferred with a minimum of 3 years of med/surg experience
3. Current CPR certification
4. Minimum of 5 years recent clinical cardiology experience preferred
5. Experience in chronic disease management and adult education principles and program development.
6. Proficient in Excel, Office Outlook, Access, Publisher, PowerPoint,
