CHP Hospice is currently accepting applications for the position of: Hospice Nurses (RNs/LPNs)
Community Health Professionals' Defiance and Van Wert Inpatient Hospice Centers offer compassionate, specialized, skilled nursing care for terminally-ill patients with comfort and support services for family members. We are currently seeking Full-time & Part-time RNs/LPNs
Our outstanding, all-nurse staff delivers comfort at times when patients cannot be best cared for at home. We offer general patient care, residential care and respite care in a beautiful 12-bed, all-private-suites center designated exclusively for hospice care and designed with comfort and family needs in mind. As part of our dedicated team of hospice caregivers, you will be part of a nurturing environment that combines round-the-clock care and a commitment to supporting physical, emotional and spiritual needs. Make a difference serving patients and families one-on-one delivering specialized nursing, related to pain and symptom management, palliative and end-of-life care.
SALARY: Competitive HOURS: Full-time & Part-time available LOCATION: Positions available in Defiance & Van Wert
Duties include: •
Assess the patient and provide professional services as indicated •
Develop the plan of care in collaboration with the physician and other health care providers •
Counsel the patient/family in meeting nursing and related needs •
Appropriately document pertinent data in the clinical record •
Provide service and/or treatment for patients requiring substantial and specialized nursing skills including end of-life issues and specialized care related to the terminally ill •
Inform the physician or other health care providers •
Participate in in-service programs as requested
MINIMUM APPLICANT REQUIREMENTS •
Graduate of accredited school of nursing •
Currently licensed in the state of Ohio •
Physically capable to lift and transfer patients •
Excellent communication skills •
Good transportation and proof of current driver's license
SPECIAL REQUIREMENTS •
Hospice experience a plus, not required. Training provided. •
Must maintain license by obtaining necessary contact hours and competency
