Nursing Supervisor RN

Mary Free Bed at Home is seeking a Nurse Supervisor for a full-time position at our Grand Rapids office.

SUMMARY: The Nurse Supervisor is responsible for supporting all field staff in the on-going management of patient care with particular emphasis on monitoring the attainment of episode goals, clinical documentation, monitoring of visit frequency to ensure the highest quality clinical outcomes for the agencys patients.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned as necessary.
  • Update Clinical Record on an ongoing basis as relates to communications, orders, re-hospitalizations, conflicts, etc.
  • Evaluate and address customer concerns, including documentation and resolution of same.
  • Communicate with marketing personnel re customer issues that arise and may affect referral source relationships.
  • Responsible for communication with clinicians and physicians when adult protective services referral required.
  • Evaluate client episode to determine appropriateness of discharges versus recertification and initiation of orders as appropriate.
  • Obtains physician orders for field staff from physicians, laboratory results and resumption of care orders.
  • Forwards LUPA information to Director of Clinical Operations.
  • Participates in staff meeting agenda and in-services.
  • Track Home Health Aid Supervisory Visits and assure that visits are being performed by appropriate field staff.
  • Monitors patients clinical records concurrently and retrospectively, utilizing agency thresholds and indicators.
  • Audits clinical records for completeness on and on going basis and works with professional clinicians to ensure compliance and comprehensive documentation.
  • Works with professional clinicians in developing the plan for patient frequency of visits that are clinically appropriate for the patient.
  • Assists agency personal in care coordination of patient/client services. Serves as a liaison between the organization, referral sources, patient/clients and agency personnel.
  • Review and correction of laboratory requests.
  • Assists in education of staff regarding hospitalizations.
  • Proficient in HCHB, HBSS and Outlook.
  • Participates in nursing recruitment dinners as needed.
  • Maintains Medicare guidelines regarding consents, ABN, non-provider coverage forms.
  • Performs field visits as needed.
  • Participates in ongoing education.
  • All other duties as assigned by supervisor.

EDUCATION and/or EXPERIENCE:

  • Must possess an Associate Degree from an accredited School of Nursing.
  • A minimum of two (2) years of experience working as a Registered Nurse including some medical/surgical experience and/or an acute care or home care setting is preferred.
  • Minimum of one (1) year of management experience preferred.
CERTIFICATES, LICENSES, REGISTRATIONS:


Must have and maintain a valid Michigan Driver's License, maintain automobile insurance coverage and have access to a reliable automobile. Must have and maintain in good standing professional license, certificate or registration.
