The Social Worker will provide social service support to patients, families, and their caregivers to cope with chronic, acute, or terminal illnesses. You will also research and refer patients, families, or care givers to community resources to assist in managing their illness and facilitate access to services such as financial assistance, legal aid, housing, home health care facilities, etc. Services include, but not limited to; educating patients and families, counseling, and making referrals for other services as needed.

Shift: 11:30am to 8:00pm

Required Qualifications:
*Bachelor’s degree in Social Work required or related field such as Psychology or Sociology.

Preferred Qualifications:


  • 1 or more years of experience working for a healthcare insurance company, drug reimbursement call center, a physician’s office, social services, or a home health care company

  • Knowledge of Medical Terminology

  • Ability to express ideas clearly in both written and oral communications, read and interpret documents such as safety rules and procedure manuals, resolve problems independently, and speak effectively before groups/customers or employees of an organization

  • Pro-active self- starter with experience in researching available services and agencies to assist clients with any issues. Strong computer skills required; data entry, proficiency with Microsoft Suite software, type at least 40 WPM.

  • Strong attention to detail along with the ability to multi-task and comply with competing regulatory requirements

  • Experience in developing and preparing action plans in addition to consulting with patients and care givers to resolve complex issues.



Education:
*Bachelor’s degree in Social Work required or related field such as Psychology or Sociology.
