What You Will Do:

As a CaseManager and as the key registered nurse on the patient care team, you willcoordinate, supervise and manage the clinical care to patients from admissionto discharge for patients in their home. You will develop, implement,coordinate and evaluate patient specific care plans according to physicianorders and provide one on one nursing care and treatment to patients.

  • Performsthe role of clinical episode management to include oversight of clinical tracksand clinical outcomes; manages multidisciplinary care coordination on caseloadfrom admission to discharge
  • Assess,evaluate, develop, implement and communicate the progress of patient's healthand condition using required documents, reports and established timelines.
  • Providessupervision and evaluation of aides and nurses
  • Leadsdiscussions on multidisciplinary patient care cases as appropriate
  • Communicatesand collaborates with all members of the patient care team; physicians, familymembers and community resources to achieve and maintain continuity of patientcare

Degree of Travel: Homevisits to cover Wheeling, WV. Periodic office meetings.

Schedule: Monday - Friday with rotating weekends

Required:

  • Currentand unencumbered license to practice as a Registered Nurse specific to thatstate the employee is assigned to work by the Company.
  • Minimumof one (1) year of clinical experience as a registered nurse (RN)
  • CurrentCPR certification
  • ValidDriver's License and reliable transportation and agency required liabilityinsurance to perform patient visits.

Preferred:

  • Bachelorof Science in Nursing (BSN) degree
  • Acutecare experience as a registered nurse with the adult population in a medical –surgical setting
  • Previoushome health or hospice experience
  • Knowledgeand experience using the Oasis assessment tools in the homecare setting
