Schedule: Full Time, Days

Tomball Regional Medical Center is currently looking for a Intake Coord. - Registered Nurse Psychiatric Unit.

POSITION SUMMARY

The Intake Assessment Coordinator in conjunction with the Program Director and/or Nurse Manager coordinates the pre-admission process and provides clinical assessments for potential patients. Also, assists with community education with clinical issues and participates with community events such as health fairs.

ESSENTIAL FUNCTIONS OF THE POSITIONS

  • Able to assess and reassess needs of patients.
  • Provides on and off site clinical assessments for potential patients to the program.
  • Works collaboratively with hospital departments to assist managing behavioral health assessment needs.
  • Participates as an active member of the multidisciplinary team.
  • Provides ongoing education to patients and families as needed.
  • Provides resources and support for patients before, during and after treatment.
  • Coordinates and participates with appropriate interested parties and physicians through the patient referral inquiry, admission, treatment and discharge process.
  • Models and trains other behavioral health team members on managing and tracking inquiry calls.
  • Tracks and reports on all key indicators related to calls, assessment, admissions.
  • This position requires the ability to travel 4-6 hours per month to assist with referral development and community education activities.
  • Actively contributes to and works toward hospital wide improvement in meeting core measures, patient safety and service excellence goals.
  • Performs other duties as assigned by Program Director.

EDUCATION AND EXPERIENCE REQUIREMENTS

  • Possess Masters Degree in Social work or Licensed RN
  • Prefer 1-3 years experience in a behavioral healthcare setting performing clinical assessments or in a clinical treatment setting.

LICENSURE AND/OR CERTIFICATION REQUIRED

  • Must be licensed to perform clinical assessments
  • Current BLS
  • Current CPI (Crisis Prevention Intervention) required within 60 days of hire
