WPCP RN Care Coordinator

The Whole Person Care Program (WPCP) RN Care Coordinator will provide intensive case management services to homeless, high risk, high utilizing Medi-Cal and Medi-Medi beneficiaries and achieve reduced total cost of care through lowering the number of emergency room visits and hospital inpatient admissions. The RN Care Coordinator will provide comprehensive case management and coordination of care and services to people enrolled in the WPCP and will provide screening and enrollment of participants, outreach, assessment, care coordination, education, patient empowerment and utilization review, including field work and home visits and use of Telehealth technology as needed. The RN Care Coordinator participates in data collection and analysis to support care management outcomes and identify performance improvement opportunities. The RN Care Coordinator builds important relationships with patients, meeting on a one-on-one basis, acting as a liaison to primary, specialty and ancillary services. The RN Care Coordinator acts as patient advocate through the continuum and is available to the patient and community based entities as a resource to facilitate communication.  Redding Searchlight .
