Responsibilities

Position Summary:

Reporting to the Director of Quality, provides leadership and vision for the Stroke/Sepsis Program in terms of operations, services and patient care, consistent with best practices and evidence based care. Facilitates the linkage between strategic business lines and provision of an effective and efficient Stroke Program. Maintains current knowledge of clinical practice in care of stroke and sepsis patients. Demonstrates/models excellence in nursing practice through the utilization of advance concepts in the areas of assessment, diagnosis, implementation and evaluation of acute stroke and sepsis patient care. Maintains current knowledge of clinical practices in care of stroke & sepsis patients. Reviews implementation of stroke order protocols and follows up as needed to ensure compliance. Utilizes current research and evidence based standards of practice to develop, analyze and coordinate a comprehensive plan of patient education activities, including goals assessment, planning, implementation, needed resources and evaluation methods to support improved patient outcomes. Collects data and assists in the implementation of practice changes as needed, participates in the analysis of data and report creation to drive improvement of both program initiatives. Assists in achievement in the provision of quality care to the stroke & sepsis patient populations. Assists in the development of appropriate policies and procedures required for the provision of effective and efficient stroke care. Facilitates initiation and utilization of Plans of Care policies and procedures. Develops and maintains collaborative partnerships with physicians, staff, peers and interdisciplinary teams to support the implementation, and on-going evaluation of care delivery. Partners with department leadership in the provision of care and follow-up of patients. Facilitates completion of staff orientation and on-going staff development in terms of stroke & sepsis program and care.

Qualifications

Requirements:

* Minimum of two years nursing experience, including care of stroke patients. 1 year experience in managerial or program development preferred.

* Valid RN license issued by the state of CA

* ACLS, BLS

* BSN

* Working knowledge of performance improvement, licensure and accreditation requirements.

* Demonstrate good capacity for teaching, organizing, planning and development.

* Good critical thinking skills.

* Strong interpersonal communication and customer relation skills.
