Client Services Coordinator
Home Instead Senior Care seeks a multi-talented, professional and caring person to join our growing company with over 100 CAREGivers and a dynamic administrative team as a full OR part time geriatric Client Services Coordinator. The Client Services Coordinator works in all activities of client service provision to ensure that the personalized care clients receive remains consistent with the standards and policies of Home Instead Senior Care.

These activities will include: intake and assessment, family caregiver support and education, networking, CAREGiver supervision and training, care management, community education, On Call duties, marketing and coordination with other service providers. Essential Functions Participate in establishing the Annual Operational Plan and achieving sales, gross profit and productivity objectives. Maintain a working knowledge and compliance with all company publications including the Operations Manual, Personnel Policies and Procedures Manual, Franchise Websites, Mission Statement and general office protocol.

Maintain professional excellence in interactions with clients, their families, referral providers, CAREGivers, office staff, and the community. Cross-train and assist wherever needed in daily office operations. Provide after-hours coverage 8 or more days per month. Client Services Provide assessment of care needs and description of services to prospective clients and family members. Develop client service plan Assist in selection of CAREGivers. Ensure accuracy of service plan and successful provision of services and care management. Maintain contact with clients and family members. Coordinate with other service providers including personnel from Home Health, Hospice, Health Care Facilities and Hospitals. Networking Increase awareness of Home Instead services among existing and potential referral providers. Perform additional marketing activities to generate awareness and stimulate service inquiries. Attend networking functions.

Training and Supervision Provide training for CAREGivers on specific topic areas. Conduct quality assurance visits. Other Responsibilities Attend quarterly CAREGiver meetings. Participate in annual Franchise-wide special events. Incidental errands. Other duties as required.

** Has a Bachelor's degree with a background in Human Services, Social Work, geriatrics, end of life care, counseling or a related field.
