As our Client Care Coordinator, your primary mission will be to insure the delivery of the best care possible for our Seniors, supporting our frontline staff, and being the center point of communications and relationships with the clients and families we serve. Your service-oriented skills and consultative approach will come into play as you determine each client's needs, recommending the solutions for care and developing the Care Plan. Your exceptional organizational skills will insure that regular client visits are performed, documented, and system updates are accurate. Primary Responsibilities:

  • You will be a key member of the Care Consulting Team, handling Service Inquires and Care Consultations with clients, families, and professional case managers.
  • You will be a member of the frontline team, creating a professional and welcoming experience at Home Instead&hellip. exemplifying our mission of providing compassionate care and support.
  • You will perform a quality assurance visit schedule to insure client satisfaction and on-going accuracy of Care Plans, supervise our home care team, problem-solving any issues and be the critical communication link to clients, families, and medical professionals.
  • Your attention to detail will insure the business enters and maintains accurate client profiles and records in the operating system.
  • You will be a representative of Home Instead in the community, engaging with the senior community and healthcare partners on a regular basis.
  • Your charitable interests will be satisfied by managing our annual holiday charity gift-giving campaign for seniors.
  • You will utilize your exceptional communication skills to keep the team updated and current on all care issues, challenges, and changes.

Education/Experience Requirements:

  • College graduate preferred.
  • One year of experience in home care, health care or senior-related industry a plus. One year of business experience or an equivalent combination of education and work experience may be considered
  • Must possess a valid driver's license
  • Must pass the Home Care Services Bureau registration and clearance process.

Knowledge, Skills and Abilities:

  • Must demonstrate excellent oral and written communication skills and the ability to listen effectively and problem-solve.
  • Must have the ability to work independently, maintain confidentiality of information and meet deadlines
  • Must demonstrate effective interpersonal skills, business development skills, as well as sound judgment and good decision-making skills
  • Must demonstrate discretion, integrity and fair-mindedness consistent with company standards, practices, policies and procedures
  • Must be patient and congenial on the telephone
  • Must have computer skills and be proficient in Word and Excel
