About this job

*Coordinates flow of information between multiple care providers during transfer/transport process.
*Ensures all necessary transfer documentation is sent and received.
*Contacts medical insurance provider and obtains authorization as needed using standard work.
*Utilizes standard work to obtain bed assignment based on diagnosis and acuity level.
*Contact referring entity with bed assignment and availability, provide phone number for nurse-to-nurse report and estimated time of arrival.
*Coordinates between Client contracting personnel, the insurer, and the referring hospital to obtain Letters of Agreement and communicates any payer issues with Medical Director or Management as needed.
*Arrange transportation per service protocols and consultation with transport RN and/or MD as needed.
*Facilitates direct admissions as requested from physician offices, clinic, or patient's home.
*Assist with bed coordination for outbound transfers.
*Documents actions and patient data into transfer center software, EHR, and other hospital clinical information systems as appropriate.
*Reviews on a regular basis the hospital transfer protocols. Communicates with management regarding any suggestions to improve the transfer process and protocols.
*Triages general public calls including, but not limited to, assisting in physician referral inquiries and general informational questions and services.
*Ability to problem-solve, triage and troubleshoot issues and escalate to management as appropriate.

Communication (Oral and Written):
*Coordinates the distribution of necessary transfer information with all necessary departments, including but not limited transport, bed control, admitting, referring entity, patient financial services, and case management.
*Contacts medical groups and health plans to obtain insurance authorization for patient with appropriate CPT and ICD codes.
*Consistently communicates with internal and external customers in a courteous, professional manner that reflects the desired service outcomes of the department and Client affiliates.
*Maintains all files per departmental standards.
*Maintains professional relationship with internal and external customers.
*Completes all necessary documentation as required.
*Emails Daily Executive Summary Report using standard naming convention.
Technical Skills:
The SHTC Coordinator works in collaboration with the SHTC Nurse for the arrangement of patient transfers/transports to and from Client Hospitals.

Quality:
*Maintains confidentiality of all patient, physician and hospital data obtained, distributed and maintained by the SHTC.
*Performs self-audits for quality and accountability of data integrity in transfer center software system.
*Review daily reports for accuracy with SHTC Nurse.
*Assures timely communication with Manager or Director to provide high quality, efficient service recovery.
*Responsible for performing ongoing quality audits related to call capture recordings, SHTC Nurse checklists, and data accuracy in transfer center software system.
*Conducts transfers in accordance with EMTALA rules and regulations.
