Care Manager RN - 6 month contract to hire

South Kansas City, MO - Near Leawood

Job Description

The Care Manager RNs will be interacting with patients over the phone and in rare instances face-to-face alongside the provider to document. They will essentially be matching the patient needs with the appropriate services, which are approved by the providers. Maintain and update patient records.

Priority of work:

60% patient phone calls;

  • Collect patient medical records
  • Determine list of possible services that might be called for (Doctor consults care manager on patient records and makes his/her decision on appropriate service)
  • Ensure delivery of care in quality, efficient, and cost-effective health care services
  • Coordination of health services, scheduling
  • Monitoring and evaluating potential options, given to provider
  • Follow protocols for individualized care plans
  • Manages caseload of patients and provides targeted interventions to avoid hospitalization and ER visits
  • Educates patients/families on health care options

40% administrative duties;

  • chart audits ("gap reporting") for payers/insurance, providing them any possible missing data
  • scheduling annual wellness visits
  • attribution lists (list of brand new patients for provider from the payer)
  • pre-visit chart planning
  • quality measures ("we're missing a colonoscopy or fall risk assessment", lets provider know what needs to be checked off the list at their visit)

Shift/Hours:

  • 8a-4:30p, but the shift is negotiable after starting, between the contractor, providers and practice manager (could look like a 7-4 shift if the provider would be in)

Qualifications

  • MO Registered Nursing license
  • 3 years' experience in the last 5 years in either primary care or home health care setting. They recently hired an RN from an overnight float pool from a hospital, and she made up for her lack in the same environment with being able to pick things up quickly, so they are open to that as well.

Computer literacy; proficient in Microsoft Office Suite (specifically spreadsheets and outlook) and internet use ("gap reporting" is done in payer portals online) - I will be testing candidates on these qualifications.

Start Date:

  • ASAP (as soon as background clears, checking to see if could start pending)

Pay - $60k-$65k/year
