What does a Patient Care Coordinator do?

As a new Patient Care Coordinator at Katy Trail Community Health, you can expect a dynamic and engaging day-to-day experience centered around care coordination

The role of the patient care coordinator on the team is to provide "planned care" for the team's panel of patients who may have been identified by diagnosis or condition, as being a member of a vulnerable population, and/or as needing transition of care from an outside facility. The patient care coordinator will address barriers to care which may affect patient compliance with their care plan including following up for important appointments. This may include but not limited to, financial barriers to care, transportation assistance, language barriers, or other concerns regarding cultural appropriateness.

You will also work collaboratively with healthcare providers, addressing inquiries and resolving any issues that may arise. Expect to handle a variety of tasks that require organizational skills and attention to detail. Your schedule will be Monday to Friday, from 8:00 AM to 5:00 PM, with the occasional evening hours to accommodate patient needs.

To thrive as a Patient Care Coordinator at Katy Trail Community Health, you'll need a diverse skill set tailored to healthcare dynamics. First and foremost, strong computer skills and proficiency in word processing are essential, enabling you to manage patient records efficiently. You should demonstrate the ability to organize and categorize information effectively, ensuring that care coordination runs smoothly. Excellent written and oral communication skills are crucial to interact with providers, patients, and third parties while applying tact and sensitivity. A solid understanding of customer service principles will help you navigate patient inquiries with ease.

Being knowledgeable about community resources, along with health-related financial assistance programs such as Medicare and Medicaid, is highly desirable. Familiarity with federal, state, and local regulations is important for compliance, while the ability to prepare reports will enhance communication. Lastly, being bilingual (English/Spanish/Russian) is a plus, as is the capability to operate basic office machinery and maintain good work habits such as attendance and accountability.

Knowledge and skills required for the position are:

  • Demonstrate well developed written and oral communication skills with ability to interact with providers and third parties, including ability to utilize tact and sensitivity to timing in personal transactions
  • Demonstrate understanding of customer services principles
  • Ability to accurately record and transmit detailed information
  • Knowledge of available community resources and the ability to evaluate resources and make appropriate referrals preferred.
  • Knowledge of health-related financial assistance, including Medicare and Medicaid preferred.
  • Ability to interpret and comply with federal, state, and local laws and regulations.
  • Ability to deal effectively with a diverse patient population.
  • Ability to prepare reports and communicate effectively orally and in writing.
  • Bilingual (English/Spanish/Russian) preferred.
