This position is responsible for the development, implementation, and evaluation of systems and programs directed toward the provision of quality services for cardiac & stroke patients from a multidisciplinary perspective across the entire spectrum of cardiac & stroke care. The Cardiac & Stroke Program Coordinator will work in collaboration with the Emergency Services Medical Director, Emergency Department and inpatient units. This individual is expected to have a clinical background with cardiac & stroke experience as well as an economic and business understanding of cardiac & stroke services and healthcare. Responsible for the overall collection and retention of pertinent data concerning cardiac & stroke patients. This responsibility includes identifying appropriate patient cases; identifying appropriate data for collection; data collection; assisting with monitoring of performance improvement indicators, data entry and retention; generating reports; promoting favorable patient relations and expressing the values of the hospital philosophy in daily work activities. The individual in this position will be required to fully understand the purpose and operation of Environmental Services along with having the ability to make decisions based on sound judgment when a situation dictates.
This is a full-time remote salary exempt position working Monday-Friday from 7:00am-3:30pm.
SPECIFIC ACCOUNTABILITIES (not limited to):
Serves as expert practitioner and role model in providing quality nursing care to the cardiac & stroke patients of all ages and their families.
• Support the team and the clinical management of the cardiac & stroke patient.
• Assist in monitoring compliance with cardiac & stroke standards throughout the hospital stay.
• Provide and document cardiac & stroke care in accordance with the standards relating to the care of cardiac & stroke patients.
• Identify the need for and coordinate the development and implementation of care paths for cardiac & stroke patient populations incorporating members from the multidisciplinary team.
• Serve as a resource for the clinical staff. Participates in education of all staff and hospital personnel relating to the care and treatment of the cardiac & stroke patient.
• Communicate patient assessment and coordination of care in progress notes and discharge summaries.
• Coordinates community injury prevention activities, and serves as a resource to the community-at-large for education about cardiac & stroke.
• Supervise adherence to hospital policies and procedures and standards through observation, medical records review, staff feedback, and other appropriate sources.
• Submission of data on a regular basis to the CARES (Cardiac Arrest Registry to Enhance Survival) program and Get with the Guidelines (Stroke)
• Identify and record quality improvement issues identified in the initial data entry of the cardiac & stroke patient record.
• Develop and provide reports of cardiac & stroke services to other individuals or committees as needed.
• Report findings to Medical Director of Emergency Services.
• Assist with monitoring of performance improvement indicators.
• Maintains professional growth and development through seminars, workshops and professional affiliations to keep abreast of latest trends in field of expertise.
• Ensures no injuries to self or others by following safe work practices and policies. This includes, but is not limited to: security and safety, understanding of MSDS, equipment, infection control, fire, disaster, safe lifting and body mechanics.
• Ensures self-compliance with organization policies and procedures, as well as labor agreements.
• Ensures the interface with team members and other support groups is conducted in a courteous and efficient manner conducive with the organization’s values.
• Conducts self in a professional manner and ensures personal appearance meets the standards necessary to perform the job function while representing the organization.
• Ensures that additional accountabilities, as may be required by management, be handled in a manner necessary to meet organizational standards.
POSITION QUALIFICATIONS (not limited to):
- License:
a. Licensed to practice professional nursing in the State of Washington. - Certification:
a. Currently possesses one of the following: Certified Emergency Nurse (CEN); Certified Neuroscience Registered Nurse (CNRN); Stroke Certified Registered Nurse (SCRN)
b. Currently possess Pediatric Advanced Life Support (PALS) and Advanced Life Support (ALS) and Basic Life Support (BLS) - Skills/Competencies:
a. Demonstrates competency on equipment listed on department specific checklist.
b. Critical thinking skills: Seeks resources for direction, when necessary. Performs independent problem solving. Decision-making is logical and deliberate.
c. Performs actions that demonstrate accountability. Exercises safe judgment in decision-making. Practices within legal and ethical guidelines.
PHYSICAL REQUIREMENTS:
- Occasional climbing and crawling/kneeling.
- Ability to lift /carry up to 25 lbs.
- Frequent walking, sitting/standing, reaching, pushing/pulling, lifting, carrying, bending/stooping/crouching, twisting/turning and repetitive movement.
- Ability to communicate using verbal and/or written skills for accurate exchange of information with physicians, nurses, health care professionals, patients and/or family, and the public.
