Direct Service Worker-Elderly Care

Haven's Gateway Personal Care Facility Inc. for over 18 years is seeking a Direct Service Worker to join our team! You will assist in the daily care of elderly or disabled individuals. Responsibilities:

• Assist clients with daily living activities on a daily basis based on the clients comprehensive Plan OF Care
• Communicate ongoing care results and updates to supervisors and written daily notes
• Collaborate with clients for the best care based on their CPOC
• Maintain a healthy and comfortable living environment
• Ensuring that the client makes all assigned appointments

​Qualifications:

• Previous experience in personal care or other related fields 6 months
• Compassionate and caring demeanor
• Ability to build rapport with clients
• Excellent written and verbal communication skills
• Background Check
• Reliable transportation
• Valid Drivers License and Auto Insurance
• Social Security Card
• 18 or older
