Job Title: Hospital Social Worker (BSW/MSW)
Location: Sitka, AK
Relocation Bonus: $10,000
Employment Type: Full-Time, Day Shifts

Position Summary
The Hospital Social Worker works closely with the multidisciplinary inpatient care team to assess and support the biopsychosocial needs of patients across all life stages. This role focuses on proactive, integrated care coordination and discharge planning, resource linkage, patient advocacy, and counseling. The Social Worker also supports swing bed admissions and collaborates with primary care providers and community agencies to ensure patient-centered continuity of care.
Key Responsibilities
Conduct biopsychosocial assessments of hospitalized patients.
Lead or assist in discharge planning, including overcoming barriers to discharge.
Facilitate or participate in patient/family care conferences.
Assist patients with financial, housing, legal, and long-term care needs.
Coordinate Durable Medical Equipment (DME) and community resource referrals.
Support patients in advance care planning (Advance Directives, POLST, etc.).
Complete swing bed admission assessments and support subacute rehab care.
Refer to licensed clinical social workers for mental health, substance use, or crisis intervention needs.
Maintain accurate and timely documentation in compliance with policy and regulations.
Participate in multidisciplinary team meetings and training.
Build trusting relationships with patients and families to support healthcare goals.
Maintain knowledge of community resources and collaborate with outside agencies as needed.
Required Education and Licensure
Education: Bachelor’s or Master’s degree in Social Work (BSW/MSW).
License: Valid Alaska driver’s license (or ability to obtain within 6 months of hire).
Experience Requirements
Minimum of 2 years’ experience working in a hospital environment as a social worker.
Must be at least 21 years of age.
