Description:
**PT, SLP, & OT placements will be required to submit the following requirements within 1 week of signing - These documents are required for the credentialing application and failure to complete these timely could result in a pushed start date - DOB, SSN, resume, transcript or diploma, education verification, & minimum of 3 references 2 peers and 1 department chair (need name and email address)**

OUTPATIENT PT

13 weeks
40HPW, 5x8hr Days 0800-1630

Required: 1yr exp, BLS, WI State License (pending licenses accepted) & Bachelor's, Master's or Doctorate degree in physical therapy.
