We are seeking a Part-Time, Ongoing Service Coordinator/Case Manager to join our team! You will provide ongoing support to our participants via monthly telephonic visits and quarterly in-person visits.

Responsibilities:

  • Coordinating multiple services among multiple providers
  • Securing annual level of care assessments
  • Ensuring team meetings are scheduled and held as designated in the service plan
  • Facilitating the acquisition, oversight, and delivery of service; Ensuring annual service plans (Revised Service Plans/RSPs) are completed in a timely manner
  • Facilitating the waiver applicant/participant’s Plan of Protective Oversight (PPO) is completed and supports the service plan
  • Conducting monthly visits via telephonic and/or in-person visits with the participant no less than once a quarter
  • Maintaining records for at least six (6) years after termination of waiver services
  • Responding to participant crises and emergencies
  • Addressing problems in service provision; and other matters as requested by participant or informal support.

Required Qualifications:

  • Master of Social Work with (1) year of experience providing case management/service coordination and information, linkages, and referrals regarding community-based services for individuals with disabilities and/or seniors or
  • Bachelor’s degree with (3) years of experience providing case management/service coordination and information, linkages, and referrals regarding community-based services for individuals with disabilities and/or seniors or
  • Associate’s degree with (5) years of experience providing case management/service coordination and information, linkages, and referrals regarding community-based services for individuals with disabilities and/or seniors.

Preferred Credentials:

  • Certification as a Peer Specialist, Community Health Worker, or related credential (a plus).
  • Bilingual proficiency (a plus).
  • Familiarity with trauma-informed care, cultural humility, and community-based service navigation.
  • Strong interpersonal, organizational, and communication skills.
  • Knowledge of community resources in Long Island and surrounding regions is preferred.
  • Experience with electronic record systems and care management platforms.
