Job Details:
• Schedule: Monday-Friday, 8am-5pm with 18 patient encounters per day minimum• Provide care for a mix of pediatric and adult patients (must see patients 12+ years, preference for providers who can see infants)• Chart closure required within 72 hours• Must have 3 years of experience in a similar setting• Active California medical license required before start• PAs must have a CA Controlled Substance Education Course Certificate• NPs must have a Furnishing License with Schedules II• Active DEA needed before start• BLS certification required• Required immunizations include: Flu Vaccine, TB Status/X-Ray Results (within 90 days before start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (within last 10 years)• Credentialing timeframe: 45-90 days• 30-day notice required for ending assignments• Nearest airport: ACV - Arcata

Compensation:
Competitive hourly rates with weekly pay and malpractice included.
