Job Requirements

American Heart Association BLS certification required for all direct patient care personnel
License/Certificate Verification must be dated within 30 days of start
Specialty exam competency testing must be completed within a year
Must provide documentation of required vaccinations and health screenings including Fit Test, Trinity 10 Panel Drug Screen, COVID-19 Vaccine, MMR, Hepatitis B, Influenza Vaccine (optional), 2 Step PPD OR IGRA, Varicella, Physical/Health Statement, Tdap vaccine (declinations accepted if no pediatric interaction)
Must complete Trinity Code of Conduct Form, Trinity I9, Confidentiality and Security Agreement, Integrity and Audit Services Form, Non Employee Upload Form, Conflict Of Interest Disclosure Form, Criminal Background Check, Education Verification, EPLS, GSA/OIG, Joint Commission Testing, NSO, System Access Form, Skills Checklist, Child and Adult Dependent Abuse Training (IA Only), IA Sing Report, OFAC
