Position Overview

We are seeking a Medical Director – Geriatric Medicine to lead a Program of All-Inclusive Care for the Elderly (PACE) in Mishawaka, IN. This is an outstanding leadership opportunity for a Board-Certified Family Medicine or Internal Medicine physician with a passion for caring for frail and elderly populations in a mission-driven, interdisciplinary care model.

Leadership & Clinical Responsibilities
  • Provide medical direction and oversight for participant care

  • Lead and supervise Primary Care Providers and APPs

  • Oversee clinical outcomes and QAPI initiatives

  • Develop and maintain medical policies, procedures, and care standards

  • Participate as a key member of the interdisciplinary care team (IDT)

  • Conduct histories, physicals, reassessments, and care planning

  • Collaborate with nursing, therapy, social work, and community providers

  • Educate participants and families on chronic conditions and health maintenance

  • Participate in team conferences and care decisions

  • Support intake evaluations and eligibility determinations

  • Assist with achieving operational and quality goals

  • Travel locally as needed

✅ Required Qualifications
  • Board Certified in Internal Medicine or Family Medicine

  • Indiana medical license (or willingness to obtain)

  • DEA (verbal confirmation) prior to start

  • CDS certification prior to start

  • BLS certification

  • NPDB Self-Query Report (within last 30 days)

  • Minimum 1 year of experience caring for frail or elderly populations

  • Prior supervisory or leadership experience

  • Clean malpractice history preferred

  • Proficiency with Microsoft Office & clinical systems

  • Valid driver’s license, insurance, and reliable transportation

  • Permanent candidates only (No locums)

⭐ Preferred Qualifications
  • Fellowship-trained in Geriatric Medicine or

  • Certificate of Added Qualifications (CAQ) in Geriatric Medicine (completed prior to start)

Compensation & Benefits
  • Salary range: $231,000 – $280,000 (based on experience)
